Whitening gels commonly contain carbamide peroxide or hydrogen peroxide, which diffuse through enamel and dentine to oxidize stain molecules. Temporary sensitivity occurs when these chemicals reach fluid-filled dentinal tubules and stimulate the pulp. Enamel is not painted white or replaced by whitening, and fillings, crowns, veneers, and implant restorations do not lighten like natural teeth.

Risk depends on the product strength, contact time, frequency, fit of the tray, exposed dentine, enamel defects, decay, gum recession, and whether the product is used as directed. Poorly fitting trays can place gel on the gums. A dentist examines the mouth before treatment because whitening an untreated cavity or cracked tooth can produce significant pain.

Use a dentist-supplied or appropriately labeled product as directed and do not extend sessions or use it more often for faster results. Reduce or pause treatment and seek advice if sensitivity persists, gums burn, or pain becomes localized. A clinician may treat cavities first, adjust the protocol, use a lower-strength product, shorten contact time, or recommend desensitizing care.

Arrange an examination for pain that lasts after whitening, a single dark tooth, cracks, cavities, gum recession, or persistent mucosal irritation. Emergency care is needed for facial swelling or breathing difficulty.
