Swollen gums around braces or another orthodontic appliance commonly reflect plaque trapped around brackets, wires, bands, or aligners.

The gingiva can become red, enlarged, tender, or prone to bleeding when plaque remains at the gum margin. Food impaction, appliance rubbing, gum disease, and infection are other possibilities.

Brush around every bracket with a soft brush and interdental brush, follow the orthodontic cleaning plan, rinse after food, and do not bend wires or remove bands.

Contact the orthodontic team for worsening pain, pus, fever, a loose appliance, or tissue cut by a wire. Facial swelling or breathing difficulty is urgent.

Ask whether plaque, irritation, or infection is responsible and whether cleaning or appliance adjustment is required.
